The Whole Crew™ is designed as an ongoing relationship that supports seniors and families as needs evolve over time.  Rather than billing separately for every issue that arises, clients participate through one annual fee.

The Whole Crew™ addresses the many different aspects and complexities of eldercare providing a variety of customized services and guidance.

Your Crew Captain 

RN / Care Coordinator

At the center of The Whole Crew™ is the RN / Care Coordinator—known as the Crew Captain who serves as the primary point of coordination between the client, family members, legal advisors, care providers, and benefit administrators. 

Services also include:

  • Access to curated resources including innovative technologies that may help clients remain safely in their homes
  • Independent, unbiased recommendations for the client and family
  • Rapid assessment of care needs, including Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) evaluations
  • Evaluation of care options and resources to guide next steps and transitions
  • Attendance at care plan meetings across care settings to advocate for the client
  • Quarterly in-person visits and monthly check-ins
  • Guidance regarding admissions and discharge planning across all care settings, Medicare rules, and healthcare system complexities
  • Recommendations when higher levels of care may be appropriate

Financial Assessment

Services include collaboration with the client and/or their financial advisor to:

  • Stress test existing retirement plans
  • Evaluate financial resources with evolving care and housing needs 
  • Eligibility for benefit programs that may help extend financial resources

Estate Planning 

Preparation and coordination of legal documents by an attorney to protect clients during incapacity and ensure their wishes are carried out at death.

Services may include:

  • Trusts
  • Will
  • Durable Powers of Attorney for Asset Management
  • Health care directives and related documents
  • Coordination of asset alignment with estate and elder law plans
  • Trust funding and beneficiary designation review and assistance

Elder Law Planning 

Guidance by an attorney across a wide range of elder law concerns. Services may include:

  • Asset protection strategies for the home and other investments
  • Review of assisted living, continuing care community, and nursing facility contracts
  • Long-term care insurance evaluation and claims guidance
  • VA Pension with Aid & Attendance eligibility for tax-free cash benefits
  • Medicaid eligibility and application for benefits
  • Navigation of complex legal and benefit systems
  • Oversight of how planning decisions affect the estate plan and distribution of assets

Life Care Strategy Questionnaire 

Whether used proactively or during a crisis, our Life Care Strategy Questionnaire helps seniors think through important decisions about aging, care, and personal priorities.

The questionnaire encourages thoughtful reflection about care preferences, housing choices, and quality-of-life goals—topics that are often overlooked until an emergency occurs.

Whenever possible, the questionnaire is completed by the senior so their voice remains central to future decisions. Family members often participate to help create shared understanding and alignment.
